About this deal

If you're taking paracetamol for a short-lived pain like a headache, you may only need to take it for a day or two.

Ibuprofen comes in a standardised 100mg per 5ml formulation, such as Calprofen or Nurofen For Children, which can be given to children aged three months or older and who weigh more than 5kg, with dosage depending on their age. Chronic hepatic necrosis has been reported in a patient who took daily therapeutic doses of paracetamol for about a yearNephrotoxicity following therapeutic doses of paracetamol is uncommon, but papillary necrosis has been reported after prolonged administration. This medicine contains 0.16mg benzyl alcohol in each 5ml which is equivalent to 0.03mg/ml. Benzyl alcohol may cause allergic reactions. Benzyl alcohol has been linked with the risk of severe side effects including breathing problems (“gasping syndrome”) in young children. Caution in newborn babies (up to 4 weeks old). Caution in children under 3 years old; do not use for more than 1 week due to increased risk due to accumulation. If you take paracetamol in the hour before or the six hours after taking colestyramine, the paracetamol is likely to be less effective. This is because colestyramine reduces the absorption of paracetamol from the gut.

Liver damage is possible in patients who have taken 75 mg/kg or more of paracetamol in less than an hour, and these patients should be referred to hospital. To avoid underestimating the potentially toxic paracetamol dose ingested by obese patients who weigh more than 110 kg, a bodyweight of 110 kg (rather than the actual bodyweight) should be used to calculate the total dose of paracetamol ingested in mg/kg. Signs and SymptomsTreatment with activated charcoal should be considered if the overdose in excess of 150 mg/kg has been taken within 1 hour. Treatment with acetylcysteine is used up to, and possibly beyond, 24 hours of ingesting paracetamol. It is most effective if given within 8 hours of ingestion, after which effectiveness declines. If required, the patient should be given intravenous acetylcysteine, in line with the established dosage schedule. Giving acetylcysteine by mouth (unlicensed route) is an alternative if intravenous access is not possible. Acetylcysteine can be given irrespective of the plasma paracetamol concentration: Symptoms of paracetamol overdosage in the first 24 hours are pallor, nausea, vomiting, anorexia and abdominal pain. Liver damage may become apparent 12 to 48 hours after ingestion. Abnormalities of glucose metabolism and metabolic acidosis may occur. In severe poisoning, hepatic failure may progress to encephalopathy, haemorrhage, hypoglycaemia, cerebral oedema, and death. Acute renal failure with acute tubular necrosis, strongly suggested by loin pain, haematuria and proteinuria, may develop even in the absence of severe liver damage. Cardiac arrhythmias and pancreatitis have been reported.

Do not take more than the recommended dose. You should also avoid taking more than one paracetamol-containing medicine at the same time, as this can easily result in exceeding the maximum dose of paracetamol. It is safe to take Lemsip cold & flu sachets with ibuprofen or aspirin. However, do not take them with co-codamol, co-dydramol or Tramacet because these contain paracetamol. Low level transaminase elevations may occur in some patients taking therapeutic doses of paracetamol; these elevations are not accompanied with liver failure and usually resolve with continued therapy or discontinuation of paracetamol. Patients should be informed about the signs of serious skin reactions and use of the drug should be discontinued at the first appearance of skin rash or any other sign of hypersensitivity.
